All applicants must follow these instructions carefully. No L.T.C. gun permits or F.I.D. cards will be issued by the Peabody Police Department unless all the information submitted is found to be accurate, and all documents have been provided.
Information and application packets are available in the lobby of the police station or via the attachment link above.

PROCEDURE FOR CHANGE OF ADDRESS:
1. Complete the information below. Please PRINT CLEARLY.
2. Make a legible photocopy of the front side of your firearms identification card or license to carry.
3. Send the following form- Firearms license change of address form and the photocopy of your license ( via certified mai l) to all of the following-
(1) The police department that issued your license- Peabody Police Department 6 Allens Lane, Peabody, Ma. 01960 ATN: Change of Address Notification
(2) The police department into the city/town where you have moved

Firearms Licensing Packet
Carefully read the additional attached application instructions. The Standardized
Commonwealth of Massachusetts Firearm License Application for New Applicants
and Renewal Applicants is the same.
All paperwork must be clearly and correctly filed to ensure timely processing
Completed applications must be deposited in the Firearms Licensing drop box located in
the lobby of the police station.
NEW APPLICANTS- do NOT include payment. You will be directly contacted to
schedule an appointment to complete the licensing process. Bring the check at the time of
appointment
RENEWALS- Include payment with completed application and 2 proofs of residency

Additional Application Instructions and
Information
New Applicants must complete a Basic Firearms Safety Course that is approved by the
Massachusetts State Police or complete a Hunters Safety Course that is approved by the
Massachusetts Division of Fisheries and Wildlife
• Provide a copy of the Certificate with the Completed Application.
• DO NOT PROVIDE THE ORIGINAL CERTIFICATE
All Applicants must provide 2 Proofs of Residency
Examples: Mass Driver’s License, State Identification, Motor Vehicle
Registration, Utility Bills, Voter Registration Card, etc.
All Applicants are required to fill out the standardized Commonwealth of
Massachusetts Firearms License Application
Payment accepted for Firearm Licenses shall be made by Money Order or Personal
Check – payable to the City of Peabody.
Fees are as follows:
• L.T.C. - $100.00
• F.I.D - $100.00 {$25.00 under eighteen)
• Law Enforcement Officials (Including Retirees) $25.00 (L.T.C. &
F.I.D.)
• Renewals for applicants over the age of 70 shall be at no charge.
Note: Any person who knowingly files an application containing false information shall be punished by
a fine of not less than $500.00 nor more than $1,000 or by imprisonment for not less than 6 months
nor more than 2 years in the house of correction, or by both such fine and imprisonment.
License to Carry Firearms (LTC)
• All first time applicants submit one of the following Firearm Safety Certificates
with completed application (see M.G.L. C140 131P)
▪ No certificate is required for a license that is restricted for STUN GUNS ONLY.
o A certificate of completion of a basic Firearms Safety Course that is
approved by the Massachusetts State Police
o A certificate of completion of a Hunter Safety Course given by the
Massachusetts Division of Fisheries and Wildlife
• This license is valid for up to, but not more than, 6 years (see M.G.L. C140, s131(a))
• This license permits the purchase, possession, and carrying of firearms, rifles,
shotguns, and stun guns as well as the possession of ammunition
• This type of license allows a person to carry a loaded and concealed firearm,
unless restrictions are implemented.
Firearm Identification Cards (FID)
• All first time applicants submit one of the following Firearms Safety Certificates
with their completed application (see M.G.L. C140 131P)
o A certificate of completion of a basic Firearms Safety Course that is
approved by the Massachusetts State Police
o A valid certificate of completion of a Hunter Safety Course given by the
Massachusetts Division of Fisheries and Wildlife
• This license is valid for up to but not more than 6 years (see M.G.L. C140, s129B)
• This license is valid for the purchase and possession of non-large capacity rifles,
shotguns, and ammunition
o Non Large Capacity refers to firearm feeding devices capable of carrying
10 rounds or less of ammunition for firearms and rifles, 5 rounds or less
for shotguns.
• This license allows for persons under the age of 18 to carry and possess chemical
propellants.
• Persons 15-18 years old must obtain an FID card to lawfully possess chemical
propellants. No safety certificate is required for chemical propellants
• Persons 18+ years old: an FID Card is no longer required to possess chemical
propellants
If you are a statutorily disqualified person as defined under M.G.L., you are prohibited from
possessing self-defense spray

PRIVACY ACT STATEMENT – CIVIL FINGERPRINTING
Authority: The FBl's acquisition, preservation, and exchange of fingerprints and associated information
is generally authorized under 28 U.S.C. 534. Depending on the nature of your application,
supplemental authorities include Federal statutes, State statutes pursuant to Pub.
L. 92-544, Presidential Executive Orders, and federal regulations. Providing your fingerprints and
associated information is voluntary; however, failure to do so may affect completion or approval of
your application.
Principal Purpose: Certain determinations, such as employment, licensing, and security clearances,
may be predicated on fingerprint-based background checks. Your fingerprints and associated
information/biometrics may be provided to the employing, investigating, or otherwise responsible
agency, and/or the FBI for the purpose of comparing your fingerprints to other fingerprints in the FBl's
Next Generation Identification (NGI) system or its successor systems (including civil, criminal, and
latent fingerprint repositories) or other available records of the employing, investigating, or otherwise
responsible agency. The FBI may retain your fingerprints and associated information/biometrics in
NGI after the completion of this application and, while retained, your fingerprints may continue to be
compared against other fingerprints submitted to or retained by NGI.
Routine Uses: During the processing of this application and for as long thereafter as your fingerprints
and associated information/biometrics are retained in NGI, your information may be disclosed
pursuant to your consent, and may be disclosed without your consent as permitted by the Privacy Act
of 1974 and all applicable Routine Uses as may be published at any time in the Federal Register,
including the Routine Uses for the NGI system and the FBl's Blanket Routine Uses. Routine uses
include, but are not limited to, disclosures to: employing, governmental or authorized non-
governmental agencies responsible for employment, contracting, licensing, security clearances, and
other suitability determinations; local, state, tribal, or federal law enforcement agencies; criminal
justice agencies; and agencies responsible for national security or public safety.
Applicant Notification and Record Challenge
Your fingerprints will be used to check the criminal history records of the FBI. You have the
opportunity to complete or challenge the accuracy of the information contained in the FBI
identification record. The procedure for obtaining a change, correction, or updating an FBI
