A major goal of the Marlborough Police Department is to make the department more accessible to the public through the use of modern technology. Part of this effort includes making the forms residents need available online to download for free. We are beginning this effort by posting some of the most commonly requested forms on our webpage.
Police Department Public Record Request (DOCX) : This Public Records Request will be directed to the City of Marlborough Police Records Access Officer.
Motor Vehicle Crash Operator Report (PDF) : If you are involved in a motor vehicle accident where someone is injured, and/or if there is over $1000 in damage to a vehicle or property, you must submit an accident report to the Police Department and MA Registry of Motor Vehicles within five days.
Registry of Motor Vehicles Complaint of Improper Operation (PDF) : This form is used to file a complaint with the MA Registry of Motor Vehicles if you witness the improper operation of a motor vehicle.
Firearms Licensing Application (PDF) : Applicants for a new firearms license or those renewing an existing license can apply either online through the MIRCS Unified Portal at www.mass.gov/firearms-services or in person by scheduling an appointment with a licensing officer. IF YOU CHOOSE THE ONLINE OPTION, YOU MUST CONTACT A LICENSING OFFICER WITHIN TEN DAYS OF FILING THE APPLICATION TO ARRANGE PAYMENT OF THE APPLICATION FEE. YOUR APPLICATION WILL NOT BE PROCESSED AND WILL BE CANCELED IF YOU DO NOT CONTACT A LICENSING OFFICER TO ARRANGE FOR PAYMENT OF THE FEE WITHIN TEN DAYS OF FILING THE APPLICATION. Application Fees: LTC - $100, FID - $100, FID Card Under 18 years of age - $25. A check or money order made payable to the City of Marlborough is the only acceptable form of payment. CASH WILL NOT BE ACCEPTED.
Request for Sex Offender Information from City/Town Police Departments: This is the applicable form for a resident to request sex offender information relative to Marlborough. The form must be presented in person with identification provided as required by the Sex Offender Registry Board.
Solicitor Permit Application (PDF) : The Solicitor Permit Application must be filled out completely and dropped off at the Marlborough Police Department. The application will be reviewed and may take a few days to process.
Application for Permit to Engage in Ice Cream Truck Vending (PDF) : This form is to be filled out by ice cream vendors to engage in ice cream truck vending within the City of Marlborough. The permitting authority shall only issue permits after conducting a criminal background investigation into the criminal history of an applicant to determine eligibility for a new permit or a renewal.
Dementia/Cognitive Issue Alert Form (PDF): This is a cooperative effort of the Marlborough Police Department and the Marlborough Council on Aging to assist caregivers of individuals with dementia or cognitive issues. The data provided is to be used to assist in the investigation of a person who is reported missing. See online submission.
Citizen Complaint Form (PDF) : If you have a complaint against the Police Department or an individual employee of this department, please ask to speak to a shift supervisor who will assist you in documenting your complaint which will be forwarded to the Office of the Chief of Police for review. Complainant will receive notice within 30 days as the status of complaint.
Public Safety Dispatcher Application (PDF) : This form is to be used by applicants seeking employment as a Public Safety Dispatcher. Completed applications should be submitted to Lt. Daniel Campbell.
