IMPORTANT LTC and FID LICENSING APPLICATION AND RENEWAL INFORMATION
All applicants are directed to mail in/drop off their application to:

Applicants shall include a check/money order (NO CASH) for the application fee, as no processing will be initiated without the appropriate fee enclosed. Checks shall be made out to the Town of Concord.
Fee
The licensing fee for LTC/FID applicants under the age of 70 years old is $100.
The licensing fee for FID applicants under the age of 18 years old is $25.
There is no licensing fee for individuals who are 70 years of age and older.
Scheduling
Applications are processed on a monthly basis. In your application, please provide an email address for scheduling purposes. If you do not have an email, then please provide a phone number.
Your completed application form (including payment) must be mailed or dropped off prior to the appointment date. Once the application is received, an email/phone call will be sent out informing you of your appointment time.
